Information Required on a Death Certificate
The following personal information is needed to fill out a Missouri death certificate:
- Full Legal Name
- Date of Birth
- Social Security Number
- Address
- Marital Status
- Veteran Information
- Occupation
- Parents’ Names (including Mother’s maiden name)
- City and State of Birth
- Highest Level of Completed Education

Processing a Death Certificate
- Once the certificate is filled out and reviewed by the family, it must be signed by the funeral director, embalmer (if applicable), and a physician.
- The signed certificate is filed with the Missouri State Department of Vital Records and the number of certified copes requested by the family are obtained by a funeral home representative and given to the family.

Purchasing & Reordering a Death Certificate
- Missouri death certificates cost $13 for the first copy and $10 for each subsequent copy purchased at the same time.
- Certifiedcopies are required for banking, personal property deeds, insurance etc. Our staff will help assist you with determining how many your family will need.
